Independent Domestic Worker Registration Request Form

This is not an employment application. Dana's Housekeeping Personnel Service ("Dana's") is an Employment Agency and Domestic Referral Agency operating as a registry. Dana's helps Professional Independent Domestic Workers find work with clients seeking housekeeping or other domestic services. Dana's is not the employer of the Domestic Worker/Housekeeping Service Provider it refers.

As an Independent Domestic Worker, you will receive a 1099 NEC form documenting your earnings every calendar year.
